Necessary documents
To make us possible to obtain the death-parish register you will need to have the following documents:
- The identity card and the address card of the deceased; In case of a foreign deceased person the passport
- The parish register of the deceased
- If the marital status of the deceased was married the marriage certificate is also required
- If the marital status of the deceased was divorced it's necessary to have the document legitimating
the fact of the divorce
- If the deceased is a widow we will have to have the death-parish register of the spouse
- The Proof of Death and the Statistical Document made out by the respective doctor
Furthermore if the death-body or the ashes are intended to be placed in an already existing grave/niche
the below records and documents can be also required:
- In the case of an already existing grave or niche the sequential number of the grave,
the name- and the exact date of the deceased already resting there is a necessary data
If the deceased was a pensioner you can make the process easier to have the following document along
(as our colleagues notify death the Pensionary Organization) if placing an order for the funeral
- Pensioner card of the deceased which incorporates the pensionary prime number
